Read the HOWTO doc - i guess /usr/doc/HOWTO/BootDisk...
it is basically
    format the floppy using fdformat
    make ext file system using mke2fs
    copying the files as mentioned in the HOWTO

or if u have he 'mkbootdisk' script, this is the easiest way to make a
bootdisk
for your system
