I have the same card...
What I did was to recompile the kernel..
However, I had to select the Crystal driver to make my OPLSAX work
I/O 220 is the SB i/o port...
for the SAX, I use
cs4232 0x534 Irq 5 drq 0
cs4232 MIDI 0x330 Irq 9 Drq 0
OPL FM 0x388
Try this...it works for me
