I assume these are SCSI disks.  As the problem occured when you added
another disk, I strongly suspect Cable/Termination.

The disk farthest from the motherboard SHOULD be terminated.  If internal,
look for a jumper.  If external, there will need to be a terminator on the
cabinet housing the drive.

All other disks on the chain MUST NOT be terminated.

In case you know what I am talking about, then I would suspect teh cable
itself.  In 4 years of Sun machines with SCSI. I never saw a bad disk,
each problem I saw was always a cable problem.  Good cables cost the
earth, though.  I have bought SCSI cables for 1200 each.

BTW, I assume you have BIOS extensions on your Adaptec(?) card, have you
tried CTRL-A and seeing if the adapter sees two disks?
